But last year, Dr. Qamar decided to abandon VIP medicine and pursue a no-frills version of his practice, charging just $59 for monthly membership to his MedLion clinics (16 locations in five states) and $10 a visit—and never billing insurance.

 

Dr. Qamar is part of a new and growing generation of concierge doctors who, in this era of health reform, see more opportunity in the middle class than they do in the jet set. The trend has bifurcated the retainer medicine industry: On one end, patients pay thousands of dollars a month for lavish celebrity-type treatment at traditional concierge practices. On the other, pared-down clinics charge roughly $50 to $100 a month for basic primary-care medicine, more accessible doctors, and yes, money savings for those looking to reduce their health spending.

 

Of the estimated 5,500 concierge practices nationwide, about two-thirds charge less than $135 a month on average, up from 49% three years ago, according to Concierge Medicine Today, a trade publication that also runs a research collective for the industry. Inexpensive practices are driving growth in concierge medicine, which is adding offices at a rate of about 25% a year, says the American Academy of Private Physicians.

These include swelling; skin irritation or dimpling; breast or nipple pain; a nipple that turns inward; redness, scalyness, or thickening of the skin of the nipple or breast  or a discharge other than breast milk. Breast cancer can also spread to underarm lymph nodes and cause swelling there before a tumor in the breast is large enough to be felt.

A South Florida plastic surgeon is defending himself from what he says are inconsistencies in the autopsy report in the death of a popular Miami DJ.

The autopsy from the Miami-Dade Medical Examiner says the cause of death for Betty Pino, known as the “Queen of Radio” in South Florida, was “delayed complications of cosmetic surgical procedure.

But Dr. Constantino Mendieta, a specialist in sculpting glutes, says he performed a reconstructive procedure, not a cosmetic one, and says the report has the name of another doctor in it.

“There are some gross inconsistencies that we are going to address with them,” Mendieta said Thursday. “In the report it stated that I removed silicone implants, I did not do that. It calls me Dr. Rivera which I am not Dr. Rivera.”

If you’re middle class and you think that your relationship with your family doctor is secure, think again.

Come January 1st when the Affordable Care Act takes effect, your doctor may not be around, or even if he or she is, you may be seeing a nurse practitioner or physician assistant during your next visit.  The health care system is about to become a game of musical chairs, and when the music stops, there’s no guarantee you’ll have a seat.

Despite the partisan debate concerning the Affordable Care Act (a.k.a. Obamacare), both sides of the political spectrum endorse one aspect of it: Patients should no longer be denied coverage because of an existing medical condition. But the mechanism in the law that makes that possible – mandating that all 33 million previously uninsured Americans obtain health insurance – also will exacerbate the already dire doctor shortage in the U.S., which is projected to be 65,000 by the end of next year. To make matters worse, the doctor shortage is particularly acute among primary care physicians (internists, G.P.’s, family doctors, pediatricians).

But few desks are optimally configured for standing while working with the top of the screen at or below eye level, leaving many standers hunched over. “For every inch your head inclines forward, you add 10 pounds of weight for your body to carry,” says Jill Miller, ERYT, a fitness therapy expert and creator of the Yoga Tune Up program. “Your spine becomes like a fishing rod with a giant tuna hooked at the end of the line–but that tuna is your head,” Miller says. The potential pain: Tightness and aches that start at the back of the neck and run down to the middle of the back or up into the head (turning into a tension headache). The pre-hab: Miller says that this Thoracic Rolling Pin exercise is one of the most requested moves from students in the Rx Series at Equinox gyms, a recovery and performance-boosting class she helped develop. Take two old tennis balls that have had most of the stiffness whacked out of them (or Miller’s Yoga Tune Up Therapy Balls) and place them at the top of your shoulder blades as you lie on your back. Interlace your hands behind your head and gently pull your chin toward your chest. Raise your pelvis off the floor so that your upper back is resting into the balls. Inch the balls slowly up and down the spine (if they don’t move easily, gently lean from side-to-side as you roll). Do this for two minutes, breathing into the balls as you roll. To loosen up your fascia, you need 90 seconds of constant pressure, Miller says. *Those with serious pain or injuries should consult an actual PT who is trained and credentialed.

How General Anesthesia Can Affect an Elder’s Mind

As a person ages, the chance that they will need to undergo surgery requiring the use of general anesthesia increases.

General anesthesia describes the combination of intravenous and inhaled drugs used render an individual unconscious and unable to feel physical pain during a surgical procedure. This type of intervention is typically used for surgeries that will take a long time to complete, cause a great deal of physical and mental discomfort, or may interfere with a person’s ability to breathe.

The process of “going under” usually doesn’t present a problem for younger, healthier individuals. After a few days, any residual feelings of grogginess or confusion typically wear off.

However, as an individual ages, the way their body processes drugs changes and their overall metabolic rate becomes slower.

“General anesthesia does carry a higher risk for the elderly population,” says Damon Raskin, M.D., a board-certified internist and medical director for two skilled nursing facilities. According to Raskin, it takes longer for an older person’s body to rid itself of the chemicals involved in anesthesia, which can in turn prolong the negative effects of the drugs.
